ANNUAL MEMBERSHIP ( 1 year from month of enrollment ) Individual: $30 \| Family: $40 \| JR: $15 \| Associate: $20 \| Sponsor: $50 \| Corporate: $50 LIFETIME MEMBERSHIP ( Apply prior to midnight Oct 31, 2012 and receive a 50% discount! ) Lifetime Individual: $250 \| Lifetime Family: $350 NEXT STEPS 1.
